Job Title: Resident Care Coordinator
Overall Responsibilities
Responsible for the training and maintaining of hands on delivery of care and assistance with activities of daily living for all residents. Provides care in a manner that meets or exceeds expectations.
Job Responsibilities

Administer emergency first aid when needed.

Coordinate resident assessments, semi-annual evaluations before admit day and every 6 months and if a significant change adds to assessment.

Resident Service Plan implementation due on admit. day and any time a significant change and every 6 months.

Assist residents in the use of adaptive equipment and train direct staff.

Train caregivers.

Provide on-the-job training and supervision of direct care staff.

Assess all potential residents for appropriateness of placement.

Communicate with the Administrator regarding any resident/family member issues, concerns or questions.

Participate in the in-service train in programs for all staff.

Ensure that the community is in compliance with assisted living rules and regulations pertaining to resident care.

Maintain a good working relationship with residents, resident family members, all staff, and the community at large.

Other duties as assigned.

Job Qualifications

Must be 18 years of age or older.

Must have prior work experience in caring for the elderly.

Must have prior supervisory experience for at least 2 years in an assisted living community or other health care setting.

Must have current CPR and First Aid certification or obtain within two weeks of hire.

Ability to read and write English and communicate effectively with residents, family members, and staff.

Ability to read, understand with facility policies and comply with them while on the job.

Exposure to blood/bodily fluids is likely.

Has the ability to do the occasional heavy lifting (up to 40 lbs).
